This is a 39 year old plastic surgery patient who came to my practice with complaints of small breasts and asymmetry. The patient wished to go from a B-cup to a full C to small D cup range, and was also concerned that her right breast looked different than her left breast.

On evaluation, I noticed that the patient had an unusual fold under the right breast which created a flatter appearance and caused an extra indentation in the skin. The right breast was also naturally smaller than the left breast.

I recommended a breast augmentation with placement of the implants through the areolas and under the chest muscles for the most natural results. I also recommended using silicone implants for the softest feeling breasts. I explained to the patient that I could improve the asymmetric fold under the right breast, but due to the skin tightness, it could not be completely corrected. I also felt that different sized implants would correct her asymmetry in breast sizes. I also suggested tightening around the areolas to improve their shape and symmetry, but this was not something that bothered this patient.

I performed a breast augmentation placing Mentor smooth, round, moderate-plus profile cohesive silicone gel implants under the muscles and through the areolas. On the right side I used a 400cc implant, while on the left side I used a 350cc implant. I also internally lowered the right breast fold in order to round out the breast and correct asymmetry. Although the skin would not round out completely, I was able to achieve a significant amount of improvement.

The patient did very well after surgery and was extremely happy with her natural and improved appearance.
