This woman is a 29-year-old mother of two who came to my office troubled by her small breasts and the excess skin that bulged in her lower abdomen. Like many of my "mommy makeover" patients she explained that both problems had appeared only after her pregnancies. This patient also has well-controlled diabetes (the device seen in her preoperative photo is an insulin pump).

Generally, I am very reluctant to perform cosmetic surgery on diabetic patients because they are at higher risk for infections and wound healing problems. However, because this patient was very attentive to her diabetes and kept excellent control of her blood glucose levels, I felt she would be a reasonable candidate for surgery. As with all patients with significant medical issues, I obtained clearance from her primary medical physician prior to surgery.

In her consultation, I explained that I felt she would be an excellent candidate for breast augmentation, and I recommended silicone implants for a more natural-looking result. Since her abdominal sagginess was limited to the region below the belly button, I felt that a mini tummy tuck would fit her needs very well.

I performed a breast augmentation through an incision located at the lower border of the areolas, and placed 325cc Mentor smooth, round, moderate-plus profile silicone cohesive gel implants under her chest muscles for the most natural result. I also performed a mini-abdominoplasty in which I tightened her lower abdominal muscles (a full tummy tuck tightens the upper abdominals as well), then removed excess skin.

The patient recovered well and is extremely happy with the new appearance of her breasts and abdomen. She is pleased with how natural the results from her cosmetic surgery look and feel, and with how they enhance the overall look of her body.
