- Complex Breast Revision
This is a 45 year old female who had breast augmentation performed out of state by another surgeon. This patient was never fully satisfied with her results, and explained that the implants "felt tight" in her chest. The patient had recently experienced a deflation of her right implant and came to my practice regarding revision and replacement of her implants.
When I saw this patient, I noted a number of important issues. She had an obvious asymmetry, as the right implant was deflated and the left was not. Also, the implants were too small for her frame, and the implant pockets were too low and far apart on her chest (she was a 38B and believed she had 250cc implants). She wanted to have a larger B-cup bra, but her main priority was to have a natural look.
I recommended changing her implants from saline to silicone (because she had experienced multiple deflations in the past). I aslo felt she would need to increase her implant size to approximately 325cc. Most importantly, I felt she would need a complete revision and reshaping of the implant pockets, and removal of old scar tissue, in order to improve the shape and natural appearance of her breasts.
I performed a breast augmentation using her old inframammary (under the breast) incisions, and placed the new implants under the muscle (where the old implants had been). I then carefully reshaped the breast pockets, enlarging them and bringing them together to create a natural round shape with improved cleavage. I used Mentor smooth, round, moderate profile silicone cohesive gel implants.
The postoperative photos were taken two weeks after surgery and demonstrated a dramatic improvement in her breast appearance. During her entire postoperative period, this patient had virtually no pain, and she was thrilled with the natural, more aesthetic appearance of her breasts.
