The patient is a 39-year-old female who presented to my office with complaints of breasts that were too large and uncomfortable for her body. The patient had undergone two prior breast augmentations by another surgeon and currently had a “lollipop scar” pattern with excess scarring and oversized areolas. The patient was also noted to have severe stretch marks of the breasts. The patient wished to switch from saline to silicone and to have perkier and more natural appearing breasts.

I felt that an implant exchange with downsizing and changing implants from saline to silicone would give her the best overall results. I also recommended internal pocket tightening to reposition the implants, and a complete redo of her lift to tighten the skin and improve her scars and areola sizes.

During surgery, I removed this patients large saline implants which were filled to 650 on the right and 675cc on the left. I replaced them with smaller Mentor high profile cohesive silicone gel implants (500cc on the right and 550cc on the left due to a preoperative asymmetry). I also performed internal pocket tightening and revised her lift. Due to excess skin, I had to convert her from a lollipop scar to an anchor scar, but the new scars were much finer and less noticable.

After surgery, the patient was extremely happy with her new breast shape and size. She was still full, but had a much more aesthetic and natural shape, and her areolas were significantly improved in appearance. She was much happier with how she looked in her clothes and bathing suits.
