This patient is a 32 year old female who presented to my medical facility with interest in revision breast augmentation and revision abdominoplasty (tummy tuck) surgery. She had undergone prior breast augmentation and tummy tuck surgery by another surgeon and was unhappy with the droopiness of her breasts and the countour and scar position of her abdomen.

With regards to her breasts, I recommended a complex revision which included switching from saline to larger cohesive silicone gel implants, changing the implants from over to under the muscle, and tightening the breast skin and raising the nipples (without detaching them) via an anchor scar breast lift.

For the abdominal contour and scar, I recommended a repeat abdominoplasty in order to tighten the abdominal muscles and reset the scar to a lower position. Because much of her skin had previously been removed in the first surgery, I explained that she would need a vertical incision in the lower abdomen to allow for proper skin closure, but I felt this would be a reasonable tradeoff to correct the old scar and residual tattoo.

During surgery I performed a complex breast revision as noted above, and exchanged her old implants (saline, 375cc overfilled to 460cc) to new Mentor high profile cohesive silicone gel implants. I used a 450cc implant on the right and a 500cc implant on the left due to a preoperative asymmetry. I also switched the implants from over to under the chest muscles and tightened the breast with an anchor scar breast lift.

After this I performed a revision abdominoplasty with internal tightening and liposuction of the love handle areas.

This patient recovered very nicely after surgery and is extremely happy with her new appearance. She has a natural predisposition to thicker, darker scarring, however this should improve significantly as time goes on.
