This is a 44 year old male professional who was interested in improving the appearance of his lower lids. Specifically, he did not like the "puffiness" of the lower lids, or the groove that ran between his lower lids and his cheeks. When he visited me at my office, he also informed me that he wanted minimal downtime, as he wanted to get back to work as soon as possible.
I pointed out that this patient had flat cheeks due to small cheekbones, which added to the full appearance of his lower lids. This can be corrected with a midface lift, or cheek implants. Due to the patients wishes for a minimally invasive procedure, I suggested a "transconjunctival lower blepharoplasty", which means removal of lower lid fat through a tiny incision on the inside of the lower eyelid, so there are no visible scars. In addition to this, I recommended injecting Restylane, which is a tissue filler, which could help to camouflage the junction of his lids and cheeks.
I removed fat from each of the patient's lower lids as described above, and I injected approximately 0.4cc of Restylane in each lid-cheek junction in order to smooth the contour. This combination of techniques clearly addressed the patient's area of concern and allowed him to go back to work with no visible scars and minimal downtime.
